CCL student annual forms need to be completed if:

• Your child was recently identified and accepted for enrollment in the Rockwood Gifted Program and will be attending CCL; or,
• Your child attends the CCL, and it is time to fill out forms for the next school year.

Submit or return as needed. Some forms require payment.

If you would like to pay online with a credit card, please use Parent Pay Online.

1. Log in to Parent Pay Online.
2. Select your child's name.
3. Type CCL in the Search bar.
4. CCL forms will appear.

If you would like to pay with cash or check, please read all forms below and return completed forms to the CCL Office with payment(s), if applicable.  If you plan to purchase supplies on your own, please bring them to the CCL Open House or on the first day of CCL. 

Every CCL student needs to complete the Transportation form. 

Transportation form for 26-27 School year

Please click on the link above to select your child's transportation method home from CCL.  This includes new and returning students.  Filling out this form lets us know how you would like to have your child(ren) transported home on their CCL day.  If you have more than one child at CCL, please fill out the form for each child.  This will help our transportation department prep bus routes for CCL.   This form needs to be filled out before July 31, 2026.
